Upon accessing the Outcomes/Questionnaires module in the Tracker™ 4 software, there are no pre-defined categories (such as Neck & Back, Knee, Shoulder Wrist, etc.) or tests (Oswestry, Neck Disability Index, etc.).
The Tracker Version 4 installation CD includes a Sample Database (in the "EXAMPLE DATA" folder) which can be used to reconstruct the Outcomes/Questionnaires data tables. CAUTION: THIS SOLUTION WILL OVERWRITE ANY EXISTING CATEGORIES. THIS MAY CAUSE PROBLEMS IF THE USER ALREADY HAS DATA ON FILE WHICH DIFFERS FROM THE DEFAULT CATEGORIES.
1) Navigate to the Tracker 4 installation CD
2) Navigate to the "Example Data" folder
3) Highlight all files with the following filenames:
Jdotcls.*
Jdothdr.*
Jdothrg.*
Jdotnam.*
Jdotrng.*
These files define the categories and names of the Outcomes/Questionnaires in Tracker Version 4.
NOTE: DO NOT SELECT ANY OTHER FILES FROM THE SAMPLE DATABASE. COPYING OTHER FILES MAY RESULT IN THE LOSS OF EXISTING DATA.
4a) Copy the files from Step 3 into the active database location.
b) If prompted to overwrite existing files, choose "Yes" or "Yes to All".
5) Run the Tracker software and navigate to "Outcomes/Questionnaires". The default categories and tests should be restored.
